Findings: Alleged Victim (AV) is a high fall risk and is care planned to have a tab alarm in his/her room for staff to use when AV is in bed or in his/her chair. On or about July 4, 2020, staff assisted AV to get ready for bed and left AV sitting on his/her couch. Staff did not attached AV's tab alarm as required. AV fell sustaining a head injury, was transported to the hospital, received stitches to close the head injury and diagnosed with a concussion. AV's tab alarm had been malfunctioning prior to the fall and needed to be replaced. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
