Findings: Witness #1 (W1) and the Alleged Victim (AV) were care planned to be monitored for behaviors by having staff available to ensure the two residents are kept separated. On or about August 31, 2020, AV got up from his/her chair and went over and tapped W1 on the shoulder, W1 replied by pushing AV to the ground. The facility failed to provide enough staff to meet the needs of the residents, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
