Findings: On or about late September 2020, the Alleged Victim's (AV) phone went missing. AV's family noticed actions on the phone that were outside of AV's ability and asked the facility about the phone, and when staff went to look for it, they found the case, but not the phone. AV's family called the phone and the person that answered said their family member purchased the phone from Alleged Perpetrator #2 (AP2), who had worked at the facility. AP2 is responsible for theft of property, which is considered financial abuse. The facility failed to protect AV's belongings, which is a violation of Oregon Administrative Rules.
