Findings: The facility failed to follow Witness 1's care plan to have one on one care and supervision according to his/her known behaviors and altercations. The failure resulted in W1 being found choking the Alleged Victim, as well as scratching, biting and punching him/her, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
