Findings: On or about October 4, 2020, the Alleged Victim (AV) and Witness #1 (W1) were involved in an altercation, where W1 pushed AV to the ground causing back pain to AV. The facility failed to properly care plan related to W1’s behaviors to keep residents safe. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
