Findings: On or about October 22, 2020, the Alleged Victim (AV) was given a double dose of his/her anti-convulsant medication, causing his/her to become dizzy and lightheaded, placing him/her at risk for harm. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
