Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls after he/she suffered two unwitnessed falls. On or about November 15, 2020, it was noted by staff that AV was in pain and staff noticed bruising on AV's shoulder on November 17, 2020. AV was taken to the emergency room on November 18, 2020 and it was determined he/she had two broken ribs and a broken clavicle. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
