Findings: The facility failed to ensure supervision and staff support regarding known fall risks related to the Alleged Victim (AV). AV had an injury fall and was diagnosed with a fractured shoulder. The facility failed to have enough staff on hand to ensure the safety of AV, ensuring that safety checks, up to 4 per hour, were completed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
