Findings: The Alleged Victim (AV) was a known wanderer and elopement risk. On or about November 17, 2020, November 23, 2020 and again on or December 17, 2020, AV exited the facility without assistance. According to documentation, there was no injury to him/her, however, he/she was placed at risk for harm. The facility failed to provide a safe environment, which is a violation of residents rights, is considered neglect of care and constitutes abuse.
