Findings: According to the documentation, the facility failed to properly care plan for Witness 1 (W1) for aggressive behavior resulting in resident to resident altercations. The failure resulted in W1 getting in a physical altercation with the Alleged Victim (AV) punching and scratching them in the face, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
