Findings: On or about December 2, 2020, Alleged Perpetrator #2 (AP2) forcefully pulled the Alleged Victim (AV) from his/her chair that AV was sitting in. Although there was no injury to AV from the incident, AP2's actions were inappropriate and may have resulted in pain to AV. AP2's actions are considered neglect of care and constitutes physical abuse. The facility's failure is a violation of Oregon Administrative Rules.
