Findings: The facility failed to follow the Alleged Victim's (AV) care plan to have a guard on the door to prevent other residents to enter his/her room and to complete frequent safety checks to ensure his/her safety. AV was found in his/her room with a blanket tight on his/her wheelchair seated next to soiled incontinence briefs, which is a loss of dignity to AV. Alleged Perpetrator #2 (AP2) was thought to have tied a blanket on AV, however, and investigation determined that no wrongdoing or abuse by AP2 occurred. The facility's failures are a violation of resident rights, is considered neglect of care and constitutes abuse.
