Findings: On or about December 16, 2020, the Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation where AV spit on W1 and W1 hit AV and were yelling and cussing at each other. AV was to be on one to one care, however, staff called in sick and no one covered the one to one care. The facility failed to provide a safe environment and follow AV's care plan, leaving W1 at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01364 $250.00 fine assessed
