Findings: Alleged Victim (AV) had a known history of falls from attempts to self-transfer. AV suffered approximately 15 falls from November 13, 2020 through January 23, 2021, exposing AV to repeated unreasonable discomfort. On or about January 29, 2021, AV fell while attempting to use the bathroom in his/her room unassisted. AV was sent to the emergency room and sustained a cracked rib. The facility failed to property plan care and mitigate the risk of falls to AV, which is a violation or resident rights, is neglect of care and constitutes abuse.
