Findings: On or about January 30, 2021 at 4:20 am, the Alleged Victim (AV) suffered a fall in his/her room, causing injury to his/her arm, head and right side. An intervention requesting grip tape placed on the floor was put into place, however, the tape was never applied to the floor. Later that evening, approximately 10:30 pm, AV suffered another fall in his/her room, was sent to the hospital and diagnosed with a head contusion. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02429 $500.00 fine assessed
