Findings: On or about February 2, 2021, the Alleged Victim (AV) was trying to go outside and the Alleged Perpetrator #2 (AP2) tried to stop AV by blocking the door so AV couldn't go out. AP2 did not have training completed and did not understand how to redirect AV efficiently, placing AV at risk for harm. An investigation determined no wrongdoing or abuse by AP2. The facility failed to provide proper training to AP2, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
