Findings: It was discovered that the Alleged Victim (AV) had pressure sores that went undetected by staff for some time and AV had significant weight loss due to not eating. AV's care plan was not updated to reflect the changes in order to ensure AV's health and safety. The facility's failures are a violation of resident rights, are considered neglect of care and constitute abuse.
