Findings: On or about February 14, 2021, the Alleged Perpetrator #2 (AP2) roughly transferred the Alleged Victim (AV) from his/her chair to his/her wheelchair, causing bruising to his/her arms. AP2 also took AV to the restroom where he/she belittled him/her by shaming him/her for being soiled after AP2 has just changed AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical and verbal abuse. The facility allowed AP2 to continue to work after this incident, placing AV and other residents at risk for harm.
