Findings: The facility failed to follow the Alleged Victim's (AV) care plan. AV needed frequent checks to ensure his/her safety, which was not being followed by staff, placing AV at risk for harm. AV also was not receiving care of the mouth or general hygiene. The facility failed to have enough staff to meet the scheduled and unscheduled needs of the residents. The facility's failure is a violation of residents rights, is considered neglect of care and constitutes abuse.
