Findings: On or about February 14, 2021, the Alleged Victim (AV) was found on the floor in his/her bathroom after sliding out of his/her wheelchair. On February 19, 2021, AV had fallen out of his/her wheelchair while self-propelling. It was known to the facility that AV has a tendency to move to the edge of the chair while self-propelling, however, AV was not care planned for staff to be aware and intervene while AV is not sitting correctly in the wheelchair to prevent falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02281 $500.00 fine assessed
