Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV was care planned for increased checks, however, a timeline wasn't in the care plan of how often. Alleged Perpetrator #2 (AP2) failed to follow the care plan for AV, by not physically checking on AV at night while on shift at the facility, leaving AV at risk for serious harm. AV was found on the floor from a fall, it was not known how long AV had been on the floor. AV was transported to the hospital for treatment and was diagnosed with a neck fracture. AP2's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
