Findings: Alleged Victim (AV) relies on the facility to maintain his/her Resident Trust Account (RTA). On or about April 02, 2021 it was discovered money from the AV’s trust account was missing. The money was taken by an unknow Alleged Perpetrator 2 (AP2). AP2 actions are considered financial exploitation as defined in OAR 411-020-0002(1)(e)(A)(E). The facility was not conducting audits per facility policy. The facility failed to protect AV from financial exploitation, which is a violation of resident rights, constitutes neglect as defined by OAR 411-020-0002(1)(b)(A)(i).
