Findings: On or about April 6, 2021, the Alleged Victim (AV) was seen outside the secured building by staff. Staff went out and retrieved AV and returned him/her to inside the facility. AV was unharmed, however, was placed at risk for harm by exiting the building. AV had exited through another residents window. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
