Findings: On or about April 19, 2021, Witness #1 (W1) punched the Alleged Victim (AV) in the face. W1 had a history of being physically aggressive, however, the facility failed to properly care plan for W1 to address the behaviors and have interventions in place to keep residents safe. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
