Findings: The Alleged Victim (AV) moved into the facility on April 29, 2021. Upon move in, AV had a few of his/her medications, but not all of them. From April 29 until May 18, 2021, the facility failed to administer AV's medication to him/her, as the facility could not get AV's medication due to insurance and the pharmacy used. Staff did not notify management of the lack of medication. AV was sent to the hospital on May 17, 2021 due to chest pains and having a heart attack, due to lack of medication. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
