Findings: The Alleged Victim (AV) is a known fall risk and was to be checked on multiple times per shift. On or about May 23, 2021, AV fell, resulting in a laceration to his/her head and a broken arm. AV was sent to the hospital for treatment. The facility failed to provide enough staff to meet the scheduled and unscheduled needs of residents. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
