Findings: On or about June 7, 2021, Alleged Victim (AV) and Witness 6 (W6) had a resident-to-resident altercation that resulted in AV sustaining a bruise/mark over his/her eye. W6 has known aggressive behaviors and has a history of resident-to-resident altercations. An investigation determined that the facility failed to protect AV from W6's aggressive behaviors by failing to take enough precautions and/or remedy any solutions prior to this incident which is a violation of resident rights, is considered neglect of care which constitutes abuse.
Sanction: RCFCP21-03645 $500.00 fine assessed
