Findings: The Alleged Victim (AV) was newer to the facility, however, in his/her short time at the facility had already been known to exit seek. On or about June 27, 2021, AV eloped the facility through an unlatched door in the common area of the facility. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
