Findings: On or about July 1, 2021,at approximately 8:00 am, the Alleged Victim (AV) was found cold and shivering in his/her room, lying on a bare mattress with the windows open in his/her room. The day prior at approximately 1:00 pm, staff had removed AV's bedding to wash it when AV was found saturated in urine. The window was opened at that time to air out the room. AV is care planned to be check 4 times per shift for safety. The facility failed to follow the care plan, placing AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03034 $1500.00 fine assessed
