Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about July 1, 2021, AV had an unwitnessed fall. The facility failed to appropriately assess AV for injuries and intervene when AV experienced a significant change of condition from the fall. The facility did not send AV to the hospital until July 7, 2021, where AV was diagnosed with a fractured hip that required surgical intervention. The failure is a violation of resident rights, are considered neglect of care and constitute abuse.
