Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations and are both care planned to be kept separated from each other. On or about July 16, 2021, AV was sitting in the dining area when W1 arrived. AV put his/her hand out and touched W1, to which he/she reacted and punched AV in the face. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
