Findings: On or about July 16, 2021, the Alleged Victim (AV) was found on the floor with an injury to his/her elbow and was taken to the Emergency Room. AV had multiple falls prior to this fall and was a known fall risk. The facility failed to properly care plan around AV's falls in order to prevent further falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03324 $1000.00 fine assessed
