Findings: On or about July 23, 2021, the Alleged Victim (AV) was found in his/her room soaked in urine and his/her bed was also soaked in urine. AV was not check on or toileted prior to shift change of day to swing shift due to staff miscommunication. The amount of urine AV was left in caused AV unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03324 $1000.00 fine assessed
