Findings: Alleged Victim (AV) relies on the facility to manage his/her medications. On or about July 25, 2021, Alleged Perpetrator 2 (AP2) did not follow proper procedure for medications and gave AV the wrong medication. AP2's actions are considered neglect as defined in OAR 411-020-0002(1)(b)(A)(ii) and constitutes abuse. The facility failed to provide a safe medication administration system which violates Oregon Administrative Rules.
