Findings: The Alleged Victim (AV) was known to have falls. AV had at least 3 falls during the month of July and was found to have bruises on his/her shins on July 20, 2021. AV was found on the floor of his/her room on July 23, 2021 and fell from his/her wheelchair on July 28, 2021. The facility failed to properly care plan to alleviate AV's falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
