Findings: Alleged Victim (AV) is care planned for catheter care and to be re-positioned/rotated every two hours. According to documentation and witnesses, AV's catheter was not properly cleaned, and AV was not re-positioned/rotated every two hours due to lack of staff. AV experienced loss of dignity and unreasonable discomfort. Alleged Perpetrator 3 (AP3) was notified of the concerns and failed to take any action and prevent AV from loss of dignity, which is considered neglect of care and constitutes abuse. The facility failed to prevent AV from loss of dignity, which is a violation of resident's rights, is considered neglect of care and constitutes abuse.
