Findings: The facility failed to properly care plan regarding known fall risks related to the Alleged Victim (AV). AV had multiple falls, ranging from no injury to injury with pain. Most recently, AV fell and injured his/her forehead and finger. AV's service plan remained unchanged after many of the falls and was most recently updated on March 9, 2021, and AV continued to have falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00244 $500.00 fine assessed
