Findings: Alleged Victim (AV) experienced a change of condition which resulted in AV experiencing unreasonable discomfort. Alleged Perpetrator 2 (AP2) failed to provide appropriate health assessment, oversight and monitoring when AV’s condition changed. AP2’s action is considered neglect of care which constitutes abuse. The facility failed to provide appropriate health assessment, oversight and monitoring when AV’s condition changed which led to AV not receiving timely medical care. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
