- Findings
- AV has a history of falls. The facility failed to properly care plan for AV's change of condition related to a fall that occurred on or about, August 8, 2021. AV's pain medication was not filled for four days after the fall, which caused AV unreasonable discomfort. This failure is a violation of resident rights, is neglect of care and constitutes abuse.
