Findings: Witness #1 (W1) has a history of serious altercations and has had an altercation with the Alleged Victim (AV) previously. On or about September 3. 2021, AV and W1 engaged in an altercation where W1 caused AV to sustain a bruise to his/her face, a laceration to his/her nose and a scrape on his/her left arm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03673 $375.00 fine assessed
