Findings: Alleged Victim (AV) relies on the facility for his/her care and AV is a known fall risk. Between July 26, 2021, and September 15, 2021, AV was exhibiting increased emotional behaviors and continually pacing around the facility. AV suffered a few additional falls between September 4, 2021, and September 8, 2021, and was diagnosed with a hip fracture at the hospital. The facility failed to intervene when AV experienced a change of condition, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
