Findings: Alleged Victim (AV) presents with impaired memory and decisional capacity. It was reported that between August 5th through October 12, 2021, Witness 1 (W1) had engaged in sexually inappropriate behaviors directed at other residents on approximately eight (8) occasions. On or about August 7, 2021, W1 began to preform sexual inappropriate behaviors in front of AV. Between August 24th and 28th, 2021, W1 exhibited multiple incidents of sexually inappropriate behaviors directed at AV. The facility failed to provide a homelike environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
