Findings: On or about September 30. 2021, the Alleged Victim (AV) had an unwitnessed fall in his/her bathroom. Staff assisted AV up and checked for any injuries and did not find any. AV was placed on alert for a non-injury falls. On October 3, 2021, AV was found to have a bruise on the side of his/her face, which is assumed to be from the fall on September 30, 2021. Facility staff did not send AV out to the hospital after the bruising was found to have him/her checked for a head injury, leaving AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
