Findings: Witness #1 (W1) was known to have violent incidents. On or about October 19, 2021, the Alleged Victim (AV) and W1 engaged in a physical altercation. AV wandered into W1's room and used W1's bathroom, W1 got very upset and hit AV in the arm. The faciilty failed to implement interventions to keep residents safe. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03650 $375.00 fine assessed
