Findings: On or about October 23, 2021, the Alleged Victim (AV) was found on the floor at approximately 7:40 am. AV was sent out to the hospital for treatment and was diagnosed with two broken toes and a broken nose. AV was care planned to be checked every two hours and at shift change. AV had not been seen by any staff since 2:00 am. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03655 $1125.00 fine assessed
