Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about October 29, 2021, Alleged Perpetrator 2 (AP2) grabbed AV's arm causing AV to lose balance and breaking his/her fall by catching him/herself on the wall. During the investigation, it was discovered that on or about October 14, 2021, AP2 also attempted to dump AV out of his/her chair. Both incidents caused AV emotional distress. AP2s actions are considered physical abuse. The facility failed to protect a resident from physical abuse which violates Oregon Administrative Rules.
