Findings: On or about December 11, 2021, Witness #1 (W1) and the Alleged Victim (AV) engaged in an incident. W1 was throwing plates and bowls. W1 threw body wash on AV, getting into AV's eyes. AV was sent to the hospital for treatment to wash his/her eyes out. W1 is known to have altercations, however, his/her care plan does not have adequate interventions to avoid harm to other residents. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
