Findings: Alleged Victim (AV) relies on the facility to provide a safe environment. Witness 1 (W1) had a known history of altercations. On or about December 10, 2021, W1 and AV got into an altercation when W1 entered AV’s room. Facility staff failed to monitor W1, which resulted in the facility failing to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
