Findings: According to the documentation, the facility failed to properly care plan for Witness 1’s (W1) known aggressive behaviors towards residents. On or about October 26, 2021, W1 got into a physical altercation with the Alleged Victim (AV) in the dining room. W1 punched the AV multiple times in the arm causing pain and emotional harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
