Findings: The Alleged Victim (AV) is known to fall and had 16 falls in 4 months. AV is care planned to have staff assistance when ambulating. On or about December 13. 2021, AV was found lodged between two chairs. AV suffered bruising to his/her shoulder, eye and hip. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
